I’ve made an MKV file with two subtitle tracks (normal + commentary). I can extract each track in a SUB/IDX pair. Is it possible to combine both tracks (as two “languages”) in one SUB/IDX pair, and how?

It’s possible, but you’ll need some tools to help you do it, and it’s way beyond the scope of this forum.
Try videohelp.com – they have a number of threads about this, or you can start your own.
You can use Subtitle Creator to do that. Open the first stream and save it to a new file, then the second and save to the same file. It will ask you to overwrite but when you say yes the second stream will actually be appended instead.
