When you select the CODE insert

Like this...

The FIRST pop-up window is normal.

But when you do it SECOND time, int he SAME MESSAGE,

like this....

 The pop-up window is small, and you have to scroll around or expand it to find the SUBMIT button.

BROKEN on Chrome Browser

OK on IE.
