Internal Drive is very slow

Four years before I bought a New Desktop computer with 500 GB Western Digital Internal Hard Disk and I have five volumes in my internal hard disk. I mean Local Disk C, New Volume E, New Volume F, New Volume G, New Volume H. Last three weeks it’s working very slow. If I go to any one of my local drive, it takes upto 15 seconds to open. I defragmented two times. After defragmented, now it takes upto 10 seconds to open. What is the actual problem ?

Data Lifeguard Diagnostic for Windows

I scanned my physical drive with WD diagnostic. After scanning, I clicked the repair button.But, it shows “unable to repair”. What can i do?

Note: It taken from Normal Mode. Yesterday I
started in safe mode. There is also a above mentioned problem in safe
mode also. In safe mode also, the local drives takes a minute to open.
The disk is always 100 % in Task Manager.
