What have your tried, and how?

I’ve tried to reformat it with different utilities: standard Windows reformatting, diskpart, Acronis… It didn’t help. It shows me an error message “Input/output device error” sometimes when I try to reformat it.

It means your hard drive is faulty. It should be replaced.
